Die Geschichte von »Blood+ Adagio« spielt im Russland Anfang des 20. Jahrhunderts, ein Jahr vor der Februarrevolution, durch die die Zarenherrschaft beendet wurde. Somit ist die Handlung deutlich vor der Zeit der ersten Manga-Serie anzusiedeln. Saya und Haji reisen – offiziell als Cellisten – an den kaiserlichen Hof in Petrograd, um die vampirartigen Monster, die sich dort in harmloser Gestalt verstecken, zu vernichten. Dort müssen sie sich auch mit dem Leser bekannten Gesichtern aus der ersten »BLOOD«-Saga herumschlagen, allen voran Sayas Zwillingsschwester und Gegenspielerin Diva …

A las puertas de la Revolución Rusa, Saya y Haji llegan a San Petersburgo buscando a Diva. Las intrigas palaciegas se mezclan con el descontento del pueblo, que recela de la mano derecha de los zares: Grigori Rasputin, una figura misteriosa que realiza extraños experimentos con humanos relacionados con los quirópteros.
