Ebenso wie sein Zwillingsbruder, Youhei, ist Kouhei Nikaidou ein Offizier der siebten Division und hat einige schwere Verletzungen überlebt, unter anderem den Verlust beider Ohren (eins davon durch Tsurumi), sowie eines Beines (in einem Kampf mit Hijikata) und der rechten Hand (Sugimoto), was seinen ohnehin schon labilen Geisteszustand noch mehr beeinträchtigt hat. Er trägt Prothesen, die von Arisaka angefertigt wurden und ihm einige interessante Spezialfähigkeiten verleihen. Durch die vielen durchlebten Schmerzen ist er morphinabhängig geworden. Nach dem Tod seines Bruders ist er besessen davon, Rache an Sugimoto zu nehmen, und er ignoriert alles, was sich ihm in den Weg stellt – gerne auch die Befehle seiner Vorgesetzten.
